Company and workforce fit

  • Which employer types and states are supported?
  • What W-2 headcount, hours, participation, or workforce conditions matter?
  • What facts trigger a specialist or manual review?
  • What would disqualify the company?

Employee access and benefit value

  • What exact care, services, or access does the employee receive?
  • Which providers, locations, networks, or virtual services apply?
  • What limitations, waiting periods, exclusions, or usage conditions exist?
  • How is this different from comprehensive group health coverage?

Current-plan compatibility

  • How does the program interact with the company’s existing group plan?
  • What plan types or arrangements require additional review?
  • Could participation affect HSA, Section 125, fixed-indemnity, or other benefit treatment?
  • Which plan, legal, and benefits documents should company advisors examine?

Payroll and financial assumptions

  • What does the employer pay, what does the employee pay, and when?
  • Which deductions, contributions, fees, reimbursements, or cash flows occur?
  • What exact tax mechanism is being relied upon?
  • What assumptions could materially change the modeled employer impact?

Implementation ownership

  • Who configures payroll and validates the setup?
  • Who manages enrollment, eligibility changes, notices, and employee communication?
  • What company resources and approvals are required?
  • What is the realistic timeline from approval to launch?

Evidence and decision protection

  • Which claims are supported by written plan documents, data, counsel, or published authority?
  • Are examples clearly labeled, representative, and inclusive of costs and limitations?
  • Can the employer receive assumptions and responsibilities in writing?
  • What happens if the company or its advisors decide the arrangement is not appropriate?
